How Dr Hannah approaches it

Dr Hannah reviews the health story already in motion: what has been tried, what has changed, what information is missing and what matters to the patient now. The goal is a focused plan, clear safety-netting, appropriate referral when needed and follow-up that keeps care connected rather than fragmented.
